Voter list revision: A quick guide on how to fill SIR enumeration form

The Special Intensive Revision (SIR), a voter-list clean-up drive launched by the Election Commission, is underway in nine states and three Union territories in its second phase. During the process, which will continue till December 4, Booth Level Officers (BLOs) will go house-to-house at least thrice for the distribution and collection of enumeration forms.

 

The states where the exercise is currently underway are Chhattisgarh, Goa, Gujarat, Kerala, Madhya Pradesh, Rajasthan, Tamil Nadu, Uttar Pradesh and West Bengal. The second phase will also cover Andaman and Nicobar, Lakshadweep and Puducherry.

 

Since there have been reports of confusion and concerns among voters on how to make sure their names are in the revised list, here is a quick guide on filling out the enumeration form.

 

  • Voters should make sure that the BLOs have given them two sets of forms. Each form has five major sections.

  • The first section on top of the form includes the BLO’s name and contact number, as well as the voter’s details. In the rightmost column, the voter can paste his or her more recent photograph, if required.

  • In the second part, the voter should provide their date of birth, Aadhaar number and mobile number. The name and EPIC number of the guardian, mother and partner can also be given here.

  • The third part is only for those whose names had figured in the last SIR voters’ list. For example, in Kerala, the last SIR was held in 2002, and those whose names had been included in the list can provide the same details here. Those who were not part of the last SIR can skip this section.

  • If the voter was not included in the last SIR, he or she should fill in the fourth section of the enumeration form. Here, the voter can mention the name of a family member whose name was included in the previous SIR. The name and number of the assembly constituency, as well as the order number, should be given as per the last SIR list.

  • In the last part, the voter or any of their family member should sign an undertaking that the information given is true and accurate and that they have not applied to include their name in the list of any other assembly or parliament constituency.

 

The BLO will inspect and sign the form and provide one copy of it to the voter. Voters can check if their enumeration form has been uploaded to the Election Commission website by visiting voters.eci.gov.in.
